Yes, the scene with Ken talking to the doctor is sexist and most likely intentionally so, bc Ken comes from a subverted society where men were created as an afterthought and as an accesory to women, therefore are not supposed to pursue any kind of aspirations besides being the woman's partner. The scene shown in the trailer probably comes after Ken learns that the real world works pretty much the other way around, and that he as a man is indeed encouraged to have a profession while women are (still) expected to define their existence around men, so it just makes sense to him to play along the sexism of it all, as it's just a fun little twist that opens new posibilities for him, like "Wait, men are the barbie in this world??? I can just go out and do stuff and be things???". Ken's sexism is not against women, Ken's sexism comes from within, in a way that it's so internalized that it hasn't even occured to him that men and women can coexist as equals and have the same ambitions, bless his dumb little heart.
